Member Application Form

When submitting your membership application, please include a copy of the following forms of ID:

  • A valid California Driver’s License
    OR 
    A valid California ID Card
  • A copy of a valid work identification badge or a letter verifying your
    current employment.
  • A copy of your most recent paystub

You must also include a check for $30, which includes a $25 opening deposit into your account and a one-time $5 membership fee. If you’d like to open a checking account as well, please include an additional $25 to open your new checking account.

All applications for membership are subject to approval, identity verification and new account processing.

USA PATRIOT Act Notice: To help the government fight the funding of terrorism and money laundering, federal law require us to obtain your name, date of birth, address and a government‐issued ID number before opening your account, and to verify the information. In certain circumstances, we may obtain and verify comparable information for any person authorized to make transactions in an account. Additional documentation may be required for certain entities, such as trusts, estates, etc. Your account may be restricted if we cannot obtain and verify this information. We will not be responsible for any losses or damages that may result if your account is restricted or closed.
